Norfolk County Council wishes to commission an offer that provides targeted interventions that includes individuals aged 50 plus residing within the specified Health and Wellbeing Partnership area (on occasion younger people may be supported through this offer, where suitable).

Individuals will have been proactively identified at a risk of falls and interventions should deliver outcomes around:

a) Aging Well, including strength and mobility training, physical exercise and movement;

b) Social connection, including participation in local community groups;

c) Information and advice to support aging well, including managing money and accessing services.

The Provider should plan to support approximately 800 people per year (per Health and Wellbeing Partnership) and must ensure they have the capacity to enable this.

Description of possible extension:

The Council shall be entitled at its absolute discretion to extend the Contract Period for a further period or periods of up to a total of 24 months.

The estimate value in this notice represents the total sum across all lots and does not include any inflationary or other uplifts. The value includes the 24 month initial term plus a 24 month extension.

Options

The right to additional purchases while the contract is valid.

The council will have the right to include additional funding throughout the duration of this contract based on future assessments and availability of additional resources. Consequently, the contract is designed to be adaptable, allowing for modifications and enhancements in response to evolving needs and circumstances. This flexibility ensures that the programme can effectively meet its objectives and deliver optimal outcomes. The funding is likely to come from, but is not limited to Integrated Care Partners for example the Integrated Care Board, District Council, etc. If funding does become available for proactive intervention work and the Council deems this contract to be a suitable fit, the Council may may increase the funding available for this contract to expand the scope of the services being delivered.

Description of risks to contract performance

Norfolk County Council has joined the government's Devolution Priority Programme.

This process has the potential to lead to the establishment of a county combined authority for Norfolk and Suffolk, with strategic powers, headed by an elected mayor. A consultation is under way.

Government has also invited participation by Norfolk in Local Government Review. This process has the potential to lead to unitary local government in Norfolk. One of these changes could happen without the other. The contract may be assigned or novated to any successor authority(ies) to Norfolk County Council or to any joint body incorporating or formed by any such successor. The County Council or a successor authority or joint body may order services on behalf of other local authorities serving any area within Norfolk's current geographical boundaries.
