The contract modification is due solely to the closing of the APMS Contract and transferring of patients to the PMS Contract with Elm House Surgery. Note that the PMS Contract is a contract without an end date. The value for the Kelsey Healthcare Limited (trading as Cator Medical Practice ODS Y02811) is based on the nationally agreed tariff, and was £1484010per annum, and for Elm House surgery is £2536251 per annum, giving a combined value of £4020261 per annum. The values for the contracts as given in this notice are annual amounts. The new arrangements come into effect on 1st January 2026.
