Description

Provision of Care at Home Service, Night Service and Extra Care Service in the Oldham Council boundaries.

The service will predominantly provide eligible care services to all residents of Oldham, reflecting the borough's diverse population, including a significant proportion of residents from Black, Asian, and Minority Ethnic (BAME) communities. Providers are expected to deliver proactive, person-centred care that is culturally sensitive and responsive to the needs of each individual within their own home. Care should address not only the person's social care-related quality of life but also their broader wellbeing, in line with their assessed needs and agreed outcomes. The service provider is required to deliver care in a manner that is inclusive, empowering, and person-centred, promoting independence, choice, and control for all service users.

Lot 1. Care at Home

Description

Lot 2. Night Service

Description

The provision of Commissioned Night Care Services for adults aged 18 and over living within the Borough of Oldham, including rural and hard-to-reach areas. The service is designed to support vulnerable adults to remain safely in their own homes during the night, promoting independence and reducing the risk of avoidable hospital admissions or residential care placements. Oldham Council expects the Provider to deliver high-quality care and is committed to working in partnership with key stakeholders, adopting outcome-based approaches in the delivery of this service. The Council seeks to commission services that are safe, person-centred, and of consistently high quality. Service users shall be supported to live as independently as possible, enabling them to exercise choice, maintain their preferred lifestyle, and make their own decisions. Services must be delivered in a manner that safeguards and protects the most vulnerable residents.

Lot 3. Extra Care

Description

Extra Care Support Services are for adults aged 60 years and over living in the six Extra Care Housing (ECH) schemes across the Borough of Oldham. The type and level of support will be based on the individual needs of each resident guided by their Care Act (2014) assessment, with an emphasis on promoting independence, dignity, and wellbeing. The Extra Care Housing schemes across the borough, are owned by the Council, with the housing management and maintenance elements managed through a PFI contract. Oldham Council expects the Provider to deliver high-quality Extra Care support and to work in partnership with residents, families, housing partners, health services, and other key stakeholders. Services should be delivered through an outcome-based approach, ensuring care is safe, person-centred, and consistent across all six schemes.
