Hospitals continue to see, in line with the national picture a surge in demand leading to hospital corridors being transformed into temporary bed space with a knock-on effect to supporting appropriate but speedy discharge of patients who were medically fit. In a response to the system wide pressure Cheshire East Council directed its Community Connectors to work collaboratively with the two NHS trusts at Leighton and Macclesfield Hospital and its Adult Social Care Team to reduce the impact on these two areas. This service specification covers all service users of the Cheshire East Council Community Support Connectors Hospital Discharge and Adult Social Care workstreams. All service users will be adults (18+), be Cheshire East residents who are patients at Leighton or Macclesfield Hospital, or for Adult Social Care Referrals, fall in the scope of Cheshire East Councils Adult Social Care team. The core service provision required as part of this commission are as follows: • Provide dedicated additional capacity to support Cheshire East Council's Community Support Connectors Team and its partners in admission avoidance, discharge and wrap around support to people leaving hospital, for 7 days. (Wrap around support refers to a holistic, person-centred approach that coordinates multiple services around an individual's needs-covering areas like housing, health, education, and emotional wellbeing-to ensure consistent, tailored support.) • Provide wrap around support to people known to Cheshire East Councils Adult Social Care teams for 7 days in conjunction with Cheshire East Council's Community Support Connectors Team and its partners. • The service will be available across Cheshire East Monday to Friday during business hours. Whilst demand may require resource diverting to a targeted area, it is expected that capacity and availability will be equitable across the entirety of Cheshire East

Employer's Liability Insurance = £10million. Is required in respect of each and every claim.

Technical ability conditions of participation
Bidders must answer Yes to point 1 below and 3 other points to pass this section.
Essential Project Specific Requirements;
1. Supported people accessing Adult Social Care first points of contact or being discharged from hospital.
2. Transported patients from hospital to their place of residence.
3. Transported people to appointments.
4. Removed large items from the persons residence e.g. sofa, bed, table
5. Repositioned household items in the persons residence e.g. sofa, bed, table
In order to pass, the Supplier must demonstrate:
1. At least one contract performed in the past 3 years that demonstrates relevant experience which includes 4 out of 5 of the characteristics outlined above in Essential Project Specific Requirements; one of which must be experience supporting people accessing Adult Social Care first points of contact or being discharged from hospital.
