Nature and extent of the modifications (with indication of possible earlier changes to the contract):
The Authority is providing additional funding, obtained through the Department of Health and Social Care (DHSC), to expand the capacity of the behaviour change service. This will increase provision in the specific delivery of NHS Health Checks as part of the DHSC-funded Workplace NHS Health Checks pilot programme in Cambridgeshire and Peterborough, as outlined in the agreed project proposal and service specification (Schedule A).Due to additional funding, the Provider is required to ensure timely and accurate data is provided to DHSC and the Authority for the purposes of monitoring and evaluation, according to the minimum dataset requirement set out by DHSC in line with Schedule O of the Contract and this Variation. The contract variation ends on 31st March 2025
