ABUHB are seeking to commission a transformational new local community based bespoke, flexible service for patients returning from low secure hospital placements and locked rehabilitation facilities. ABUHB are seeking to commission a provider to work in conjunction with local statutory services to support individuals to step down from out of area and/or secure hospital placements into their own tenancies initially receiving high intensity community support.

The commissioned provider would co-ordinate the acquisition of self-contained tenancies (providing a real home for people) in individual properties for a small group of individual clients at any one time. It will then provide a bespoke package of care for each individual which could involve up to 24/7 support, with a stepping down of intervention as the person settles back into their community.

ABUHB are running a two year pilot (with the option to extend for a further two years) using the principles of an evidenced based intervention called Structured Clinical Management to work with people with complex mental health needs (who are likely to have received or to be eligible for a diagnosis of personality disorder). As part of this work, the project aims to repatriate 7 people with complex personality disorders for Low Secure mental health services into their own accommodation in the community with intensive support.
