A service to support Adult and Young unpaid and informal carers in Knowsley. The contract is commissioned by Knowsley Council. The contract duration is 01 February 2025 to 31 January 2030 with an option to extend for a two further twelve-month periods to 31 January 2032.
The service Provider/s will be responsible for delivering some statutory duties towards carers, namely the provision of formal carers assessments and parent carer needs assessments, on behalf of the Council, alongside informal support for local carers. The service will support adult carers and young carers.

Lot 1 – Adult Carers Service (18+)
A community-based service that will:
• Ensure adult carers who are caring for other adults in Knowsley are identified
• Ensure adult carers can receive a carers assessment under the statutory duty of Knowsley Council
• Ensure adult carers can receive a parent carers needs assessment.
• Ensure that adult carers who have been assessed have a support plan which is reviewed on at least an annual basis
• Provide personalised information and advice to adult carers and support to access a range of support and services that meet their identified needs
• Deliver a programme of activities that will ensure a range of support is available to carers in Knowsley that reduces the negative impact of caring on them
• Work in partnership with Primary Care Networks (PCN’s) and GP practices in Knowsley to support them to develop effective processes to identify and support carers
• Ensure carers have the opportunity to access specialist support that addresses the needs of the cared for such as dementia
• Promote carers health and wellbeing by working collaboratively with other businesses, service providers and organisations to:
o Identify carers early in their caring journey to improve outcomes
o Promote the health and wellbeing of carers including mental wellbeing
o Facilitate education and training sessions for carers
o Facilitate self-managing carers peer support groups and befriending opportunities
o Support discharge from hospital and the reablement process
o Provide opportunities for a break
• Operate the service on a Borough-wide basis

1. To provide targeted, diversionary, positive activities for young people who are carers to develop their self-esteem and resilience and offer positive activities and emotional support.
2. To provide access to information, advice and guidance about services for them and their families. This will include the signposting young people to the statutory Young Carers Assessment Team (if originally referred by another source such as education, health). This will provide the opportunity for young people to access support available from the statutory Young Carers Assessment Team.
