The contracting authority (Southwark Council) wishes to contract with suitably qualified and experienced provider/s of residential care services, across four Council owned residential homes. The services to be provided include residential, dementia and reablement residential services required by frail older people. The four homes will be grouped into two lots (two homes per lots) and bidders are invited to bid for one or both lots.

The contracting authority's primary objectives of the residential care services are:

• To provide person centred care and support to its most frail and vulnerable older people in a residential setting.

• To provide care and support for older people living with multiple and complex conditions, with some having limited mental capacity.

• To operate within a complex multi-agency health and social care community.

• To build a relationship with the contracting authority based on trust, shared values, priorities and with strong collective leadership and governance.

• To provide reablement support as required.

• For the provider to be an important part of the contracting authority's integrated care system and work closely with partners on the referral processes and pathways, to ensure safe transition of service users into residential care and/or their homes.
