This contract is for the Dementia Advisor Service. The Dementia Advisor (DA) Service will be required to offer pre and post diagnosis support, advice and information to people living with memory problems/dementia, their families, and carers across County Durham.

The Dementia Advisor (DA) Service will be required to offer pre and post diagnosis support, advice and information to people living with memory problems/dementia, their families, and carers across County Durham.

The expected start date for the proposed contract is 1/4/2022.

The expected end date for the proposed contract is 31/3/2025.

The initial duration of the contract is 3 years subject to the right of the Council (at its sole discretion) to exercise its option to extend for a term or terms to be determined by the Council (at its sole discretion) but for an additional overall term of no greater than a further 3 years from the actual end date. The maximum length of the contract is therefore 6 years.
