Hampshire County Council (HCC) is seeking a suitably qualified and experienced organisation to carry out person centered, strengths-based assessments and reviews, along with associated support planning and any additional assessment work arising under MCA or CHC, in accordance with Care Act 2014 principles and in line with HCC's policies.
This work is to be completed either in person with the individual being assessed/reviewed or on a remote basis (video call). The provider will use Hampshire provided IT devices and access to Hampshire's case management system.
The provider will provide service users with information and advice based on Hampshire resources and policies and work with people's extended support networks to reduce the need where possible for commissioned care services being used to meet assessed needs and outcomes.

The County Council are tendering the services via an open tender under the Light Touch Regime - Part 2, Chapter 3 Section 7 of the Public Contracts Regulations 2015.
Within the initial term, 500 care assessments are required.
Thereafter, should subsequent assessments/reviews be required, and it is mutually convenient to provide these, the estimated total contract value is £600,000 (including possible extensions).
