The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes.

The service shall deliver upon this aim by providing:

• An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services.

• Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence.

• Support to people to manage their mental health symptoms and challenging behaviour.

• Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate.

• Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users.
