THFSS assists vulnerable people to develop or maintain their independence within their own homes and community and where possible preventing the use of more institutional forms of care and support and reducing rough sleeping and homelessness.
The service will provide floating support to individuals requiring support because they are vulnerable and are in crisis or who have a support need. Individuals will be aged over 18 years with a broad and complex range of support needs covering the spectrum of Vulnerable Adult client groups and link in to age-appropriate services. It is the expectation that the floating support provision will be both holistic and flexible, meeting the needs of those requiring medium level support through to those requiring more specialist support with complex needs.
