• Support service users who would otherwise face unnecessary, prolonged hospital stays or avoidable admissions to acute care, long-term residential care or high-level independent care packages.

• Provide a setting for an intermediate level of care over a short time spell which minimises the risk of a person being re-admitted to a hospital due to an untimely or poorly planned discharge to their own home.

• Facilitate and allow for continuing supervision in a more appropriate setting when an acute hospital bed is no longer required.

• Provide a service that is time-limited, normally lasting no longer than 6 weeks and often much less.

• Provide, on the basis of a documented, structured, individual care plan, input that maximises independence and enables service users to live at home as independently as possible.
