"The services to be provided under this contract are for short term, discharge to assess, care home placements for patients being disharged from hospital via pathway 2.
The contract is intended to be awarded to an existing provider following direct award process C.
The contract is intended to cover services provided from 1st April 2024 to 31st March 2025, and the approximate lifetime value of the contract is between £0 and £1,201,500, dependant on the number of spot purchase placements made."

Details of the award decision-makers: The decision to award was made by a MDT consisting of representatives from Commissioning, Finance, Quality Assurance and Contracting.
Declarations of Interest were sought from the members of the MDT prior to consideration of contract award. No declarations of interest were declared.
