two.2.4) Description of the procurement
NHS North East London Clinical Commissioning Group (NELCCG) are looking for a Trusted Assessor service provider (to care homes) in Barking and Dagenham, Havering and Redbridge (BHR) with the expertise of working at a senior level in social care delivery. The purpose of the role is to undertake assessments and re-assessments to facilitate safe and timely discharges from hospital into residential/nursing care homes.
This will help avoid delays in discharge from Barking, Havering and Redbridge University Hospitals NHS Trust (BHRUT) and neighbouring hospitals. NHS Trusts and local authorities that have adopted the trusted assessor model show that it can improve the experience for the patient and reduce delayed transfers of care.
The Trusted Assessor Service has as its primary function to facilitate discharges to improve patient experience, clinical safety and patient flow to care home's. The service predominantly supports the smooth transition for patients from hospital into a care home setting, however, other discharge destinations are applicable. The service will operate 5 days a week.
This service provision will ensure timely assessments are completed for people who require residential and/or nursing provision following their hospital or Enhanced Assessment Bed (EAB) admission thus ensuring a safe and timely discharge from hospital and EAB providers.
The benefits of the service will be:
• a more timely and effective service to patients and residents in BHR
• an effective and efficient process for care homes as it will reduce the number of hospital visits
• a reduction in the time required for the completion of associated paperwork
• a reduction in discharge delays
• the assessments in a manner that offers confidentiality, respect, dignity and privacy to the Service User
The successful provider will primarily work with BHRUT and EAB providers in BHR to facilitate approximately 600 discharges from hospitals and EAB providers to care homes per annum. This will be either a discharge to a short-term EAB; a package re-start to a long-term bed, or a new placement in a care home. The successful provider will also act as an independent and unbiased assessor who will support the acute hospitals, care homes and adult social care to provide a high-quality service to patients.
The authority is seeking a three (3) year contract with the option to extend for one (1) year.
