NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU) on behalf of Norfolk and Waveney Integrated Care Board (ICB) (The Commissioner) would like to notify the market of an opportunity to provide primary medical services under an APMS Contract to the Greater Norwich population at Norwich Practices Health Centre, co-located Walk-in Centre Services and the Vulnerable Adults Service (VAS) through the Inclusion Health Hub.
The Commissioner is looking for one provider to deliver the following 3 main components under a single APMS Contract.
1. Primary Medical Services in Norwich : Actual patient list size as of 1st April 2024 was 10,983.
2. The Walk-in Centre (WiC) in Norwich: Level of activity is circa 68,000 contacts per annum
3. The Vulnerable Adults Service -Inclusion Health Hub
The total contract price for each of the individual 3 services (GP, WIC and VAS) have an affordability envelope which are as follows;
1. GP: Must not exceed the maximum affordability envelope of value of £1, 529, 315
2. WIC: Must not exceed the maximum affordability envelope of value of £1, 944, 000
3. VAS: Must not exceed the maximum affordability envelope of value of £522, 530
The service commencement date is 1st April 2025 and the estimated annual value of the contract is £3,996, 069 and will be subject to an annual uplift which is to be determined.

two.2.4) Description of the procurement
The purpose of this contract is to provide resilience into the Norfolk and Waveney health system and the successful bidder will have to prove how they will reduce health inequalities and increase integration amongst the various other healthcare providers in Norwich.
The successful bidder will need to demonstrate how they intend to work and integrate with the ICB and system partners across the ICS (including patients and their representatives) to develop the service to meet the needs of the population in line with the strategic vision of the Integrated Care system.
The primary medical services will be open in line with the national core hours from 08:00-18:30 Monday to Friday, the Walk in Centre will be open 7 days a week 07:00-21:00 and the Vulnerable Adults Services -Inclusion Health Hub 9.00- 17:00Monday to Friday.
The provider will be expected to support the Commissioner to deliver an outcomes-based approach for patients and the local system, which will, in partnership with other members of the ICS, contribute to the four key themes in our Integrated Care Strategy, which are:
- Driving integration: Collaborating in the delivery of people-centred care to make sure services are joined-up, consistent and make sense to those who use them.
- Prioritising prevention: A shared commitment to supporting people to be healthy, independent, and resilient throughout life. Offering our help early to prevent and reduce demand for specialist services.
- Addressing inequalities: Providing support for those who are most vulnerable using resources and assets to address wider factors that impact on health and wellbeing.
- Enabling resilient communities: Supporting people to remain independent whenever possible, through promotion of self-care, early prevention, and digital technology where appropriate.
The contract length will be 3 years with the option to extend for a further 2 years to provide primary medical services to the population, which covers Greater Norwich, and enables the development of the service in the context of integrated system working, as outlined in Next-steps-for-integrating-primary-care: Fuller-Stocktake-Report, and other strategic developments across Norfolk &Waveney, within that contractual period.
