It is anticipated that the proposed solution will provide the following that will have a major benefit for ‘people’, such as:
• Data shared, of particular benefit in a crisis scenario;
• Joined up health and social care services to provide more care for ‘people’ at home and in the community, avoiding the need for a hospital visit;
• Improved communication across services.
It will deliver to a health and care professionals, at the point of care, all of the information that is relevant to the care or treatment needs of that patient/citizen (people). Record access will be quick and easy, from the professional’s own system – no need to sign on to a different system.
Meeting the demands of greater efficiency and improved quality requires a transformational approach to the way services are delivered: a vital enabler of this transformation is better information and communication technology.

Our vision is that mental health service users receive outstanding care and are supported, as far as possible and appropriate, to receive therapeutic intervention, assessment and support at home. This means being supported by a team who have access to all relevant health and care information quickly, easily and in as near to real time so they can proactively keep people well both physically and mentally and can intervene early where there are risks of crisis, escalation or other clinical risk. We want every service user to have confidence that when they tell their story all of their care providers will have access to it so they don’t need to say the same thing over and over again. Currently, data integration is the biggest challenge for system partners due to numerous EPR and clinical documentation systems. Joined up data and Population Health Management tools can enable this integrated, proactive vision of care.
