two.1.4) Short description

Hammersmith and Fulham Council tendered for a service to provide care and support for residents with learning disabilities within a supported living setting. The housing consists of eight self-contained new build flats, three of the eight are accessible for adults with physical disabilities.

The provider is to supply flexible support in line with a ‘core and flexible’ model to ensure that residents are able to access support appropriate to their needs. This includes: enabling a smooth transition for residents into their new homes, managing fluctuating support levels and maximising resident’s independence.

The successful provider is also expected to co-produce the service with residents, their families and the local community in line with H&F’s own expectations of a strengths-based approach to social care.

The service will be delivered in Shepherds Bush at Emlyn Gardens, as detailed in the service specification. Providers will be expected to work with landlords to provide a housing management function and would need to enter into the appropriate agreement with the landlord to facilitate this. There are eight units available as part of this tender. The Council is looking for a provider who can deliver additional capacity, for example through additional properties or developments within the pathway. The initial contract period is three (3) years, with the option to extend for a further one (1) year and one (1) year.
