two.2.4) Description of the procurement
North Central London Clinical Commissioning Group (NCL CCG) under Barnet Directorate historically commissioned Elmstead House block contract. This historical contract is over 10 years old and in place to deliver a range of accommodation and support services to provide personalised nursing care for clients from the local area who are living with dementia or other complex mental or physical health needs. The block is for 30 beds at a weekly cost of £966.00 per bed. with a total annual value of £1,506,960.
Elmstead House is a registered Nursing home managed by the Care UK Group, and provides care and support for up to 50 clients with a range of care/support needs. These include clients with dementia-related illness, physical disabilities and function mental health difficulties. The CCG have utilised this service for +10 years and use the beds for CHC eligible clients, discharge to assess arrangements and mental health act section 117 clients. In line with the response to the Covid-19 pandemic, the beds were also used to support rapid discharges from acute hospitals in order to avoid delayed transfers of care.
This VEAT is to extend the block contract arrangement with the provider Elmstead House Care Home (Care UK). This block contract will provide a cost effective local provision of Nursing home beds for clients requiring this level of care across NCL.

Explanation:
There is a clear justification why NCL CCG are not in a position to carry out a competitive tender for this accommodation.
This historical contract is over 10 years old and in place to deliver a range of accommodation and support services to provide personalised nursing care for clients from the local area who are living with dementia or other complex mental or physical health needs.
NCL CCG currently have individuals placed in the block beds and there would be significant risks to both the individuals residing at the care home and the provider as follows:
•Clinical risk's as the CCG may have to re-home existing clients in the home, which would have a negative effect on their mental/physical health.
•Litigation and complaints from existing clients and their relatives, as the service is seen as a 'home for life' by some.
•Financial loss to the service, which may ultimately lead to its ongoing operation being unsustainable, and potential home closure.
•Increased weekly costs for the current commissioned beds as they move to a 'spot purchase' arrangement. Alternative provision we have to source would be less value for money for the CCG. Current rates under the block are better than market average.
