These services will be delivered to adults with varying needs who may have learning disabilities, physical disabilities, mental health difficulties or acquired brain injury and those who are experiencing difficulties associated with using drugs and/or alcohol and those with forensic histories.

Supported living offers a community-based option for adults who meet the eligibility criteria for whom low level visiting support or residential care home placements are not appropriate to meet assessed needs.

Support will include personal care, housing support, and social care. It will be personalised, based on an assessment of the individual’s strengths and needs and will enable people to achieve individual outcomes. Wherever possible, support will include helping and enabling the Supported Person to become as empowered and skilled to undertake tasks for themselves but there may be a requirement at times for staff to undertake tasks on behalf of the supported individual.

Providers will be required to offer creative, innovative and alternative ways of supporting people to achieve outcomes by maximising technology enabled care opportunities wherever possible, ensuring affordable and effective service delivery.
