Service to provide in-County Initial Health Assessments for Children and Young People in Worcestershire, see the following service outline
• pilot to test effectiveness of a CQC organisation utilising GMC registered Doctors to undertake Initial Health Assessments for Children Looked After in Worcestershire.
• Delivery of the pilot must be compliant with Statutory time frame for completion of assessment and use CORAM BAAF templates
• Assessments must take place face to face in Worcestershire
• 4 individual assessments per week at a Worcestershire clinic setting to be identified by HWHCT
• Cost per assessment to include all aspects of the assessment including completion of appointments, documentation, letters, referrals, individual care plan and identification of necessary follow up.
