NHS South West London Integrated Care Board is inviting suitably experienced and capable providers to apply for the Procurement for the Provision of Supporting independence in Croydon.

The service is to provide personalised care and support to adults who are frail and vulnerable, and at risk of requiring input from urgent and emergency care services and therefore would benefit from proactive, preventative planning; and/or support to improve self-management and access to voluntary and community services. This will enable individuals to receive the right health and social care to meet their needs, with the service model focusing on improving self-management wherever possible.

The service will work with residents to support identification of goals and actions to improve well-being and the ability to maintain independence for as long as possible.
