two.1.4) Short description
A new Extra Care Scheme on Eastman Village (aka Harrow View East) comprising of 60 flats, offices and communal areas is due to go ‘live’ early 2023. The citizen will have a tenancy with Octavia Housing (the Registered Housing Provider) and the Care and Wellbeing provision will be tendered by Harrow Council.
The eligibility criteria is citizens aged 55 and over with frailty, physical and/or sensory disabilities and/or dementia and/or mild-moderate learning disabilities and/or enduring mental health needs who have been assessed as meeting the National Eligibility Criteria (Care Act, 2014) and who are either ordinarily resident within the London Borough of Harrow or are the responsibility of Harrow London Borough Council (the 'Council') for supporting their social care needs. There will be a mix of citizens including those with complex needs.
The aim of the scheme is to provide an outcome focussed strengths-based model of care that supports citizens to live independently for as long as possible. This will be provided through a 24/7 Core Service with 2 waking night staff and a Flexi/Planned Care service.
