two.1.4) Short description
The Lincolnshire Wellbeing Service is preventative, it aims to;
- improve or prevent the deterioration of individuals health, wellbeing and overall quality of life;
- enhance independence, improve individuals' ability to self-care and access appropriate supporting structures and community resources;
- reduce or delay escalation to statutory support services.
The current service provides support for those experiencing ill health and living with disability, supports people with financial advice, changing medical needs, bereavement and loneliness. As a preventative service, it also keeps people out of hospital and other urgent services by improving access to support and community resources.

The current service consists of six components:
- Assessment (4 or more triggers for an individual to be eligible)
- Generic Support (4 or more triggers for an individual to be eligible)
- Hospital and care in-reach
- Small aids and adaptations (4 or more triggers an inidividual to be eligible)
- Resettlement
- Telecare Emergency response
Recent additions to the service have included:
- Hospital Housing Link Workers (HHLWs) now called Hospital and Housing Development Officers
- Support to emergency response, including localised flooding and Ukraine resettlement
The service seeks to support people make improvements in the following outcome areas:
- Manage money
- Participate actively in society
- Make or maintain social contact
- Improve physical health
- Improve mental health and wellbeing
- Reduce substance misuse
- Improve independence
- Stay safe.

Lincolnshire County Council (LCC) is in the process of assessing the best way in which to re-commission its Wellbeing Services. To that end we are seeking to understand the market’s preferred approach to a number of important issues to inform the decision-making process.
If you are interested in feeding into this initial market engagement process, the completion of a market engagement questionnaire would be appreciated. The questionnaire contains a series of questions upon which we would like to understand the providers preferred approach to the delivery of these services.
