The Learning Disability hubs will strive to achieve high quality outcomes for the people they support through timely and appropriate access to information, advice, support and/or signposting. Services will be accessible via a variety of formats
including face to face, online (websites), virtual or via the phone. How the service is delivered should be in line with individual requirements and be flexible to change at any point.
The Learning Disability Wellbeing Hubs will work with other local providers to compliment service provision, support community inclusion, and prevent duplication of services and support transition away from other commissioned support.
If individuals attend the Hub service and are in receipt of social care and support, it should be for an identified desired outcome (that has not been identified to be met in the individual's care and support plan through any commissioned support the individual is in receipt of) and the individual would exit after this has been achieved.

The contract term will be for 3 years however the Council reserves the right to extend the contract by further periods not exceeding 24 months (24 being the maximum available extension period) at the discretion of the Council based on the contract performance of the successful applicant.
