two.1) Scope of the procurement

Bromley Tailored Dispensing Service

two.1.4) Short description

The Tailored Dispensing Service (TDS) was commissioned in 2018, through an AQP exercise and contracts were awarded for 3 years with an option to extend for 2 further years, which has now been enacted.

In order to access the TDS service, a referral is sent to the TDS pharmacy from the Bromley Medicine Optimisation Service (BMOS). BMOS will have received a referral from a GP, hospital, community service, or other member of health or social care staff, who feel that their patient would benefit from having a reasonable adjustment aid dispensed.

BMOS comprises of a holistic assessment, usually in the patient's own home, of the patient's medicines routine to identify and address medicines compliance issues.

The Bromley TDS is provided by 40 community pharmacies. The contract is activity and defined tariff based. The annual contract value is £308,000. This value will be inclusive of all activity, consumables, tariffs, and all associated management fees for both the Lead Provider and the TDS Contractors

The TDS currently provide a service that:

• Supports patients to manage their own medicines and maintain independence.

• Improves patients' understanding of their medicines and improve adherence.

• Undertakes clinical medication review of patients identified as having a medicines-related problem.

• Reduces the number of inappropriate drugs prescribed using evidence-based tools.

• Reduces the number of adverse drug events experienced.

• Reduces medicines waste.

• Works collaboratively with other health and social care professionals to support and implement personalised tailored interventions for individual patient needs.

• Continuously works with the ICB and social care to develop and implement guidance/policies on medicines administration support.

• Reduces the risks of harm caused by medicine regimens or inadvertent reconciliation errors.

This VEAT is to inform the market of the ICB's intention to direct award a contract to the South East London Community Pharmacy Fed.

The Fed is to provide a management role and will sub-contract to individual community pharmacies, ensuring continuity of current service provision and providing an opportunity for new TDS providers to deliver services as a sub-contractor to the South East London Community Pharmacy Fed.

four.1) Description

four.1.1) Type of procedure

Award of a contract without prior publication of a call for competition in the cases listed below

Explanation:

The rationale for the direct award to the South East London Community Pharmacy Fed is based on the following service requirements/criteria:

• Local provider within the London Borough of Bromley.

• The South East London Community Pharmacy Fed is an organisation across south east London with community pharmacy contractors being members.

• All pharmacy contractors in Bromley are, will be, or are eligible to become members of the South East London Community Pharmacy Fed and thereby TDS providers.

• All pharmacies will have a choice of whether they want to be full members or affiliated members:

 Full members - actively engaged in the South East London Community Pharmacy Fed

 Affiliated members - part of the South East London Community Pharmacy Fed just to provide services.

• Therefore, the commissioned lead provider must already have existing established relationship with the pharmacies in Bromley to ensure continuity current arrangement and minimise any disruptions to supply.

• The lead provider must have a detailed understanding of community pharmacy services, the NHS England community pharmacy contractual framework, and future developments for community pharmacy.

• The lead provider must have a detailed understanding of risks, issues and opportunities within community pharmacy in south east London and Bromley

• The lead provider must be able to establish and manage sub-contracts with all current TDS providers, as well as community pharmacy providers in Bromley that wish to start TDS and can meet the service requirements.

• The lead provider must be able to engage all Bromley community pharmacy contractors (both TDS and non-TDS) in communications, education and training opportunities.

• The lead provider must be able to collate all TDS activity, finance and performance data, collated from PharmOutcomes and report the aggregated data to the Commissioner.

• The lead provider must ensure that all reporting templates are uploaded and updated on PharmOutcomes

• The lead provider must be able to submit all necessary reporting and invoicing within the agreed timescales
