two.2.4) Description of the procurement
NHS Cornwall and Isles of Scilly Integrated Care Board’s (ICB) vision for all commissioned services is that they should be of high quality, effective, and led by demand, need and the desired outcomes for people in Cornwall. One of the ICB’s main priorities is to ensure everyone in Cornwall can start well, live well and age well in vibrant and supportive communities.
A big part of this is about making sure people can access good care and support when they need them to stay at home, or to be discharged from hospital when they are ready to return home. Place based integrated community reablement is key to this.
We are seeking providers who can offer additional reablement capacity across the county.
Reablement is a time-limited model of ‘care’ provided at home, typically no longer than 6 weeks with an ethos of ‘doing with’ rather than a ‘doing to’ approach, which supports individuals aged 18 years or over to retain or regain abilities, confidence and skills following an injury or illness, in order that the person may continue living the life they choose to live.
Aspects of life supported by reablement could include:
• Personal care tasks such as washing, dressing, and going to the toilet.
• Domestic tasks such as food and drink preparation, cleaning, shopping, and doing laundry.
• Using transport and accessing the community.
• Mobility – moving safely and confidently around the home or outdoors, climbing stairs, getting up from a chair, getting in and out of bed and getting on and off the toilet maximising opportunities to socialise, to start or resume hobbies.
• Support overnight.
This tender is to seek an innovative, Care Quality Commission (CQC) registered provider to offer additional reablement across the whole of Cornwall. There will be some specified areas that will require a targeted approach where capacity is currently lacking. The provider will become part of an existing integrated reablement project called Home Together. As a partner in this project the provider will be expected to be responsive and adaptable as the project’s partnership flexes its service offer according to local demand and the continual process of service improvements.
