two.2.4) Description of the procurement at the time of conclusion of the contract:

To offer all Leicester, Leicestershire & Rutland (LLR) patients a choice which includes end-to-end Integrated care pathways for routine elective care that are delivered within Leicester, Leicestershire & Rutland through an Alliance partnership agreement.

Outcomes: Demonstrate the ability and flexibility to achieve each of the following key outcomes -

1. Flex according to the needs of individual patients quickly and efficiently and to offer them choice (including, where appropriate, out of County): demonstrating effective dialogue with and response to patients.

2. Innovate & continually improve whole care pathways & quality outcomes in LLR: demonstrating clinical quality, innovation, safety & patient experience.

3. Realize maximum cost savings/efficiencies to the LLR health economy including use of fixed assets: demonstrating efficiency.

4. Minimize delays to implementing effective integration (end to end pathways and shared care): demonstrating ability to bring partners together including social care and collaborate successfully.

5. Actively shift care closer to home: demonstrating local care for pathways and localities where clinically appropriate and financially viable.

6. Enable commissioners to keep a lead role and effectively hold providers to account: demonstrating accountability.

7. Deliver the Better Care Together strategy of the local health economy: demonstrating managerial feasibility and capacity

The Alliance Contract partners work together to deliver specific strategic outcomes described above. Since the original contract was awarded, referral triage has been added to the service provided in order to support delivery of the strategic outcomes above.

The services within the contract include community based elective care daycase, new and follow up outpatients in:

General surgery ; Endoscopy; Orthopaedics; Ophthalmology; Ear nose and throat; General medicine; Elderly care; Diabetic management; Dermatology; Rheumatology; Cardiology; Gynaecology; Gastroenterology; Urology; Plastic surgery; Neurology; Nephrology; Maxillo/facial surgery; Pain management; Paediatrics; Diagnostics including imaging.

The modification contract value is £29,140,152.

The total contract value including the modification will be £192,889,028

The nature of the services will continue as provided prior to the modification. The majority of the services are subject to patient choice and national payment terms. The modification is for provision of these services for an additional year, therefore affecting the duration and value of the contract.

Nature and extent of the modifications (with indication of possible earlier changes to the contract)

This modification is to extend the existing contract for 12 months effective from 1st April 2021 to 31st March 2022 to the incumbent providers to maintain service provision, consistent with PCR 2015 Regulation 72(1)(b).

The increase is due to the tariff and choice rules which limit commissioner ability to contain spend.

The modification contract value is £29,140,152.

The total contract value including the modification will be £192,889,028
