two.1.4) Short description

Solihull's hospital discharge service, called the Home Discharge Service (HDS) and admission avoidance service, called Early Response Service (ERS) have both been running since 2020 and 2021 and the contracts are due to expire in September 2023.

Both services provide care and support to people in their own home with a significant focus on maximising people's independence. HDS supports people who have been discharged from hospital and ERS supports people to prevent admission to hospital or long-term care.

As the contracts are due to expire in September 2023, we are issuing this Invitation to Tender, with award to be for two separate contracts for ERS and HDS to two separate care providers.
