The service is available to patients aged 18 and over with a registered GP in the SNEE area.

This service sits within the stroke and neuro rehabilitation suite of services to help support those who are suffering the affects of a stroke.

Comprehensive Support for Stroke Survivors and Carers:

• Extends support from the acute stroke unit to one year post-stroke, or longer based on patient needs.

• Provides a single point of contact for stroke survivors and carers to navigate the health and social care system effectively.

• Includes independent needs assessments and ongoing support for patients and carers.

Improved Health Outcomes:

• Maximises rehabilitation potential for stroke survivors.

• Promotes education to prevent stroke recurrence and manage complications like urinary tract infections, constipation, and falls.

• Supports lifestyle changes to reduce risk factors such as smoking and obesity.

Emotional and Social Support:

• Enhances emotional recovery and confidence for stroke survivors.

• Establishes long-term support groups for ongoing community engagement.

Cost Efficiency for Healthcare System:

• Reduced reliance on GP services by stroke survivors who receive proper support.

• Research indicates cost pressures of £48K per year per patient if rehabilitation potential is not maximised.

Data Collection for Future Service Improvement:

• A fully functional outcomes framework monitors progress and collects cross-organisational data for future developments.

Alignment with NHS Long Term Plan:

• Supports higher intensity rehabilitation models, ensuring better outcomes for stroke survivors and promoting equity across healthcare systems.
