The aim is to procure a range of services for carers that supports the Council to fulfil its statutory duties in respect of carers under the Care Act 2014 and Children and Families Act 2014 by ensuring they are identified as early as possible and are able to access:

• Information, advice and support

• A carers assessment and support plan

• A range of personalised support and services

• Advocacy support

There are 4 Lots:

A community-based service that will:

• Ensure adult carers who are caring for other adults in Liverpool are identified

• Ensure adult carers can receive a carers assessment under the statutory duty of Liverpool City Council

• Ensure that adult carers who have been assessed have a support plan which is reviewed on at least an annual basis

• Provide personalised information and advice to adult carers and support to access a range of support and services that meet their identified needs

• Operate the service on a city-wide basis

• Provide free, independent, professional issue-based advocacy support

• Support all adult carers (18 years +) who are caring for other adults over the age of 16, plus family carers who are caring for children with additional needs

• Promote self-advocacy to enable carers to advocate on their own behalf

• Operate the service on a city-wide basis

Lot 4 – Carers Health and Wellbeing Service

A community-based service that will:

• Deliver a programme of activities that will ensure a range of support is available to carers in Liverpool that reduces the negative impact of caring on them

• Work in partnership with Primary Care Networks (PCN’s) and GP practices in Liverpool to support them to develop effective processes to identify and support carers

• Promote carers health and wellbeing by working collaboratively with other businesses, service providers and organisations to:

o Identify carers early in their caring journey to improve outcomes

o Promote the health and wellbeing of carers including mental wellbeing

o Facilitate education and training sessions for carers

o Facilitate self-managing carers peer support groups and befriending opportunities

o Support discharge from hospital and the reablement process

o Provide opportunities for a break

• Operate the service on a city-wide basis
