The CHC team commission High Needs packages for people who have complex health needs.The HBC complex care service will include the provision of trained care staff who have appropriate skills and competencies to meet an individual’s assessed health needs. The service may also include using specialist equipment and stock control of ancillary supplies, prescription management as required by the individual and set out in their care plan. Individuals may have a range of complex health needs and/or a significant disability and/or an additional mental health need that impacts upon their daily lives. The service may also be required to support families/carers.
