"The RPCS accepts referrals from all system stakeholders for EOL patients whose preferred place of care or preferred place of death is at home. The service provides a package of care (POC) based on assessed needs of the patient as soon as the referral is accepted and based on the assessed needs of the patient. RPCS completes a checklist assessment between week 6 – 8 to ensure that the CHC team can complete the decision support tool (DST) assessment for any patient requiring fast track provision.
RPCS works collaboratively with the CHC team to ensure that any patient requiring fast track support will be managed and in line with the service delivery model.
This service helps to reduce the LOS at Acute level ensuring expedited discharge home/to the community. The service also helps to manage patients that require any additional specialist support in the community hence reducing further conveyances to hospitals and signpost patients and families to other outpatient or wellbeing services that are available. This supports the ICB strategic target for 25% reduction for non-elective admissions for frailty and EOL patients. "

This is a Provider Selection Regime (PSR) confirmation of contract award notice. This contract has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award.
