two.2.4) Description of the procurement
The Service is a short-term assessment model that enables identified individuals to remain at home by means of the provision of a care at home service. The Service Provider will play a leading role in determining whether the supported individual can continue at home or will require their ongoing health and social care support to be provided in an alternative location, typically within hospital or a care home setting. The Service will have a focus on enabling the supported person by helping them to regain their skills and confidence so they can continue to live as independently as possible when well. As part of the dynamic assessment the Service Provider will identify any longer term social care support needs the service user has. The personal outcomes of individuals and the agreed service outcomes will be central to the Service that is provided
