Explanation:

Age UK Nottingham currently provide services on behalf of Nottingham University Hospitals NHS Trust (NUH) Health Care of Older People (HCOP). The service provides the provision of independent advocacy, support, information and representation for older people and their relatives/carers, (where appropriate) during their stay on the Authority's Health Care of Older People (HCOP) wards or through attendance at HCOP clinics.

Continuity of Patient Representative service delivery with incumbent supplier is essential to ensure NUH Patients receive the support they need. Without Age UK Nottingham, NUH HCOP would not be able to provide a Patient Representative Service for its patients.

This will stop patients making informed decisions, ensuring their voice is heard, representing their interests and refer on to appropriate services as required in order to support their discharge at home, and/or to help them to remain as independent as possible, for as long as possible, in a community setting.
