two.1.4) Short description

The service to be provided is a range of personal care and support services to individual adults in their own home. Individuals will have a range of care needs from low to complex, including challenging behaviours. The service does not include supported living and services of a highly complex nature or the provision of nursing care or medical tasks that require a trained medical professional. The service will allow individuals to be supported through periods of recovery or enablement to maximise or maintain their level of independence, reduce social isolation and also provide support to carers. The support provided within Services will be personalised to meet the needs and choices of the individual. Luton Council will be shifting from time and tasks to an outcome model where two distinctive set of outcomes are measured; Individual Outcomes and Service Outcomes.

All Providers must be Care Quality Commission (CQC) registered and located in or within a short distance to the borough of Luton.

The Service must be available 24 hours per day, 7 days per week, and 52 weeks a year including bank and public holidays. Providers must ensure that sufficient cover is available over all holiday periods.

The award process will place successful Applicants onto an Approved Providers List.

The Approved Provider List will be limited to 12 Providers. 4 Providers within each Tier.
