Description

NHS Humber and North Yorkshire Integrated Care Board (hereafter the Contracting Authority) is seeking to commission Support at Home (Domiciliary Care) Services - North East Lincolnshire through an Open procurement process.

This procurement seeks to ensure the delivery of four distinct service lots to meet the needs of adults requiring Support at Home (Domiciliary Care) across North East Lincolnshire (NEL).

The Contracting Authority wishes to establish three (3) Providers for Lot 1 consisting of three (3) Geographical Lead 'Locality Providers'; and one (1) Lead Provider per each of Lots 2-4 consisting of: one (1) Lead Provider for Enhanced Support at Home; one (1) Lead Provider for Carers Breaks; and one (1) Lead Provider for System Resilience.

1. Geographical Areas (Lot 1 (a, b, and c))

• Delivery of high-quality domiciliary care services to individuals living within their specified geographical area.

• Implementation and development of the 'Teams not Times' model that improves responsiveness, flexibility and person-centred support.

• Includes management of existing extra care housing (2 schemes) (where applicable) to support independent living.

• Services must focus on maintaining independence, reducing risk, improving the quality of life, and maintaining people in their homes for longer.

2. Enhanced Support at Home (Lot 2)

• Provision of intensive support to individuals with complex care needs.

• Includes promoting health resilience and avoiding unnecessary hospital admissions.

3. Carers Breaks Provision (Lot 3)

• Delivery of flexible respite care to support informal carers.

• Must include planned and emergency breaks with tailored services to meet carers' and service users' needs.

4. System Resilience Provision (Lot 4)

• Support during peak times and emergencies.

• Services include short-term interventions to stabilise individuals and facilitate timely discharges from acute settings.
