NHS Norfolk and Waveney Integrated Care Board seek competitive offers for the Provision of Central Norfolk Reablement and Recovery Pathway: Non-weight bearing rehabilitation and bed based intermediate care.
The key objectives of the Competitive Process are specified below:
To commission 4 beds in a single suitable Care Home setting within 5 miles of the Norfolk & Norwich University Hospital, to deliver a Reablement and Recovery pathway with a primary focus on individuals who require a period of non-weight bearing, where their care needs cannot initially be safely met at home, and who require a period of reablement, to enable discharge home/Usual Place of Residence (UPOR) or least restrictive environment, following a hospital admission. For the avoidance of doubt, if a bidder wishes to submit bids for more than one care home, a sperate bid must be uploaded for each care home.
To provide personalised care for all individuals accessing the service enabling them to recover and regain their independence outside of a hospital environment with the aim of returning home/UPOR with support if needed, or to a least restrictive environment
