Tower Hamlets council aims to improve the health and wellbeing of residents. Public Health Enhanced Services delivered within primary care in Tower Hamlets are a key way to achieve this. These services encompass a range of health interventions aimed at improving population health outcomes, specifically targeting areas such as sexual and reproductive health, cardiometabolic disease prevention, and substance misuse.
Tower Hamlets General Practitioners (GPs) have long-established provision of a range of Public Health enhanced services to address substance misuse, provide for sexual health and undertake NHS Health Checks. Locally these are called Network Improvement Services (NIS). These services are in addition to the GP core contract commissioned by NHS England. The enhanced services provided include:
GP Sexual Health and Reproductive Services (SRH)
NHS Health Checks (NHSHC) programme
Primary Care Substance Misuse Services, including shared care for OST and screening and referral related to alcohol harms.

Tower Hamlets council intend to proceed with the re-procurement of the Public Health GP Enhanced Services contract (HAC6120), commencing 1st July 2026 for a duration of 2 years and 9 months, with the option to extend for up to three additional one-year periods. The total potential contract value is £6.82 million, fully funded through the Public Health Grant.
The contract covers the delivery of three essential public health services within primary care settings across Tower Hamlets:
NHS Health Checks
Sexual and Reproductive Health
Substance Misuse
These services are mandated or strongly recommended under the Public Health Grant conditions and are critical to addressing local health inequalities and improving population health outcomes.
Following a comprehensive service review and market engagement, the recommended procurement route is via the Most Suitable Provider under the NHS Provider Selection Regime (PSR). This route is justified by:
Limited market capacity to deliver borough-wide services
The need for integrated delivery across all GP practices
Planned improvements to contract structure and management
The new contract introduces a multi-layered model comprising a Core Service (responsible for service management and coordination) and Specific Services (delivered directly or via subcontractors). This structure enhances accountability, enables performance-based payments, and supports continuous service improvement.
The procurement timetable is aligned to ensure mobilisation by July 2026, with robust risk mitigation strategies in place.
This proposal represents a strategic investment in preventative health, aligns with local and regional priorities, and offers strong value for money through activity-based pricing and improved contract oversight.
