Description

This tender is applicable to Residential Care Homes located within the boundaries of the Borough of Stockton-on-Tees. The Care Home will be registered with the Care Quality Commission for the regulated service - Accommodation for persons who require nursing or personal care, with the specialism of learning disabilities.

It is not applicable to Personal Care Providers who provide domiciliary services in people's homes (This is covered by the Council's existing Care at Home Framework).

Tenders may be submitted by Providers who have CQC registered Residential Care Homes located outside of the Borough of Stockton-on-Tees, who would be interested in developing new Residential Provision for people with Learning Disabilities, at a future date.

Description of possible extension:

2 x optional extensions of 12 months may be taken.

Lot 1. Residential Care (permanent, short stay, respite) for both people with moderate and complex LD and / or Autism needs (mixed provision).

Description

Residential service can support people with both moderate and complex needs, with identified models of care and support to ensure services are safe and sustainable. The home can identify the number of beds for moderate and complex. The environment can support people with physical impairment.

Description

Residential service can support young people with both moderate and complex needs, with identified models of care and support to ensure services are safe and sustainable and support is time limited to support the young person to independent living. The home can identify the number of beds for moderate and complex. The environment can support people with physical impairment.

Description

Residential service can support older people with both moderate and complex learning disability needs plus age, related health and care needs (including dementia), with identified models of care and support to ensure services are safe and sustainable. The home can identify the number of beds for moderate and complex. The environment can support people with physical impairment.

Services will be delivered in line with Service Specification

Description

Short term residential service can support people with both moderate and complex needs. The beds will be available only for the use on this framework for people assessed as requiring:

• Planned and/or emergency respite.

• Crisis respite beds; and / or

• Time to think provision.

Services will be delivered in line with Service Specification

Framework operation description

Prices will be negotiated, based on the care needs of individual placements, using information from the Care & Support Plan of individuals and information provided by the Care Home Provider.

The choice of Care Home will be based on Service User/Service User Family/Advocate choice, based on the suitability and location which best suits the individual's residential care needs, based on the knowledge of a multi-disciplinary team who have involvement with this cohort of prople.
