Primary care review/advice for chronic disease management which will include, but not limited to, as below:

• Primary care asthma care & reviews from trained practitioners

• Primary care COPD care & reviews from trained practitioners

• Primary care diabetes care & reviews from trained practitioners - to support in-patient staff.
