The Sussex Community NHS Foundation Trust ("the Authority") sought to procure and collaborate with a provider that can resource General Practitioners across the SCFT footprint to work closely with the Trust’s UCR teams.
The expectation is to embed the GPs within each UCR team with support broken down in a hybrid model of ‘On site’ face to face working, ‘Virtual’ support and ‘On Call cover to each UCR team base they are allocated to, each with an expectation to be able to do any urgent patient visits at the patient’s place of residence (within Sussex).
This requirement is paramount to the delivery of the pathway to keep acute patients safely out of hospital and at the place they call home.
The Authority intends to award a contract, following a Most Suitable Provider Process, in accordance with The Health Care Services (Provider Selection Regime) Regulations 2023 (“PSR 2023”).
