Post-stroke 6 month reviews. Aligning the provision of 6-month reviews to an existing post-stroke service to;

• Provide a more seamless service and less fragmented pathway

• Patients are more likely to be familiar with the service and to have accessed the service as part of their post stroke care

• Support a smooth transition from a data management perspective

Stroke Association currently delivers post-acute stroke care in the Wakefield District and has built up a strong working relationship with the local therapy team, from whom it receives 200 referrals per annum.

These 200 referrals represent the patient cohort that the Commissioner is specifically seeking to target to ensure that each is offered a 6-month review, post-stroke. Individuals within this cohort are most likely to require, and value, a 6-month review, as they are those receiving post-acute care to aid their recovery from a stroke.

four.1) Description

four.1.1) Type of procedure

Negotiated without a prior call for competition

  • Extreme urgency brought about by events unforeseeable for the contracting authority

Explanation:

The proposed contractor, the Stroke Association, currently delivers post-acute stroke care in the Wakefield District and has built up a strong working relationship with the local therapy team, from whom it receives 200 referrals per annum.

These 200 referrals represent the patient cohort that the Commissioner is specifically seeking to target to ensure that each is offered a 6-month review, post-stroke. Individuals within this cohort is most likely to require, and value, a 6-month review, as they are those receiving post-acute care to aid their recovery from a stroke .

The subsequent rationale is that the proposed contractor holds a unique position to be able to deliver this care from the 01/04/2023, following the decision of the Local Authority in December 2022 to decommission a contracted provider from undertaking these reviews.
