Contract Overview

Service Description

The service will provide specialist housing-related support to prevent homelessness and maximise independence for people with a learning disability or other vulnerability

1 - Building the capacity and ability of individuals or households to maintain a home by helping them to develop the skills and confidence to live independently in a safe and suitable home.

2 - Enabling service users to exercise more choice and control in their lives

3 - Enabling the service user to liaise with other professionals or access specialist support as required.

4 - Improving the sustainability of a person’s housing by tackling their social isolation and helping them to play a full part in their community by enabling service users to develop social networks and opportunities for peer support and take advantage of employment/ leisure/ education/training opportunities as appropriate.

5 - Enabling service users to manage their finances, maximise their income and reduce debt and/or rent arrears

6 - Enabling service users to better manage their physical and mental health

The service shall run for an initial 5 years with the option to extend for a further 2 years in yearly increments.
