two.2.4) Description of the procurement

NHS West Yorkshire ICB is looking to commission Intermediate Care Nursing Bed provision for Calderdale Cares Partnership, in accordance with the NICE guidelines NG74.

The service is for people who require intermediate care support for a short time in a community bedded setting (typically no longer that 6 weeks) to help them recover and increase their independence. This service is for people aged 18 years or over who need support after discharge from an acute inpatient setting, virtual ward or to help someone in a crisis (step up service from home), to help them rehabilitate, re-able and recover, before returning home to their usual place of residence. The therapy input will be provided by Calderdale and Huddersfield Foundation Trust and is not in scope to this procurement.

The number of people referred to the service is approximately 160 people per year with an estimated population growth of 2-3% expected each year.

The provider will provide nursing care provision to people who meet the eligibility criteria for the service. The provider will provide a safe and comfortable environment, offer personalised care tailored to individual needs, enhance people’s quality of life, encourage social interaction and participation activities and support people to maintain their independence and dignity.

The service will operate 24 hours a day, seven days a week. Access will be seven days a week between the hours of 8am and 6pm.

The provider will work in collaboration with several key partners including CHFT therapy teams and Calderdale social work teams as part of a key multi-disciplinary team approach to delivering the best outcomes for people using this service.
