The Social Wellbeing service will improve the health and well-being in the district. The Service will have 2 key

priorities; to increase health and well-being and promote self-management.

The Service will deliver the following direct measurable outcomes to improve health and well-being in the local

population as a whole:

- Self-reported health and well-being is improved.

- People are equipped with the confidence and tools to have a good life.

- Knowledge and uptake of services to increase well-being, self-management and independence is increased.

- People feel less isolated and have more opportunities for social interaction and participation.

- People stay independent for longer due to increased access to information and better coordinated services.

The Social Well-being Service will improve the health and well-being of the district. The Service will have 2 key

The Service will commence on 1.4.2017 and will be put in place for 4 (four) years; therefore the expiry date will

be 31.3.2021. The Council may extend the Contract by up to 24 months, making the final possible expiry date

seven.2) Information about modifications

seven.2.1) Description of the modifications

Nature and extent of the modifications (with indication of possible earlier changes to the contract):

Modified the Contract so that it now requires the Service Provider to continue to manage the social prescribing link workers assigned to each of the Primary Care Networks ("PCNs") in Wakefield during the extended term, in consideration for an additional £80,995.00

seven.2.2) Reasons for modification

Need for additional works, services or supplies by the original contractor/concessionaire.

Description of the economic or technical reasons and the inconvenience or duplication of cost preventing a change of contractor:

A change in contractor cannot be made for economic and technical reasons. As set out in the Relevant Circumstances (above), PCNs were formally established across England from 1st July 2019 with an obligation on PCNs provide its patients with access to a social prescribing service. This was after the initial procurement, at which time the establishment of PCNs was unforeseeable and the Council's requirements in respect of the Live Well Wakefield Service managing social prescribing link workers assigned to each of the PCNs were unknown. The Service Provider is responsible for the provision of the Live Well Wakefield Service until 31st March 2023. During the extended term until 31st March 2023, it is fundamental that the Live Well Wakefield Service and the social prescribing link workers are effectively integrated and managed together to ensure service delivery is effective (with access to established infrastructure, reporting mechanisms and experience). It is not economically or technically feasible for another contractor to do so and notwithstanding which, for reasons as explained above, competition is very likely to be absent.
