NHS Arden & Greater East Midlands Commissioning Support Unit (AGEM CSU) on behalf of NHS Hertfordshire & West Essex ICB wish to inform the market of a Market Engagement event to engage with providers regarding the up-coming process for Enhanced Health in Care Homes (EHCH) in East & North Hertfordshire.

two.2.4) Description of the procurement

The Commissioners would like to invite interested providers to a Market Engagement Webinar taking place on Thursday 31st October 2024 at 13:30pm via MS Teams. The event will cover the service specification and proposed PSR process.

If your organisation is interested in attending this event, then please register your interest in attending this webinar by 1700hrs on Tuesday 29th October 2024 by following the instructions given in the additional information section below.

Key points of the service are as follows:
The Commissioner is seeking an East & North Hertfordshire Primary Medical Care Services Provider to lead the contract. Service must be Primary Care led, in collaboration with local system partners, to deliver an integrated Enhanced Health service in all care homes (residential and nursing), as registered by the Care Quality Commission (CQC), supported by East and North Practices, and which are covered under the PCN DES/DES plus. Inclusive of non-weight bearing and Discharge to Assess beds (DTA) as well as Learning disability adult care homes. Exclusive of short stay respite services, neurological services, and flexi care schemes.

The aims of the service include:
• Improvement in quality of life for residents living in care homes.
• Increase and improve implementation of Advanced Care plans (ACP) including Recommended Summary Plan for Emergency Care and Treatment (RESPECT) and Treatment escalation plan (TEP).
• People to be cared for in their preferred place of care (PPC) and to die in their preferred place (PPD)
• Partnership model delivery centred on primary medical care working collaboratively with all system partners to avoid duplication.
• Ensure ICB pathways are implemented and utilised.
• Identify best practices and the need for care home staff training and signposting to the relevant services.
• Identifying interventions needed such as nutrition, hydration, oral health care and wider primary care services including structured medication reviews and opportunities for reduction in the medication where appropriate.
• Efficient Data sharing and monitoring using the appropriate SNOMED codes.
• Shared care records updated in order that the resident’s record is visible to system partners for clinical care.
• Reduce avoidable admissions to secondary care.

This is purely an early engagement exercise and does not commit the commissioners to carry out any process. Any subsequent process will be advertised separately and all organisations wanting to participate will need to respond to the notice as and when published. Current services will remain in place during this period in line with existing contracts and service level agreements. The process will be advertised in accordance with the requirements of the Health Care Services (Provider Selection Regime) Regulations 2023.
