Support self-care for service users with Dermatology conditions including digital tools, advice, management plans and onward referral/signposting to health & wellbeing and voluntary services;

Provide rapid access to specialist advice and guidance, clinical assessment, diagnostic investigations (teledermatology), diagnosis and treatment ensuring that patients are navigated to the right pathway for their care in line with the agreed MSE Levels of Care

Deliver initial assessment and management providing a one stop model of care, where appropriate, with face-to-face provision across a number of community locations;

Deliver follow up care via patient initiated follow up model with telephone, virtual, face to face and supported self-care or shared care with primary care (where shared care pathways are agreed);

Robust management of pathways, including waiting lists, utilising a Patient Tracking / Management system.

Provide clinical leadership and case management where appropriate ensuring that care is co-ordinated across the pathway;

Participate in Multi-Disciplinary Team (MDT) meetings with primary, community, secondary and social care as required;
