two.1.4) Short description

NHS Devon ICB ("the Commissioner"), are seeking providers to deliver Primary Care Services to Homeless and Vulnerably housed populations of Exeter and Barnstaple. The service will provide the full range of primary medical care services through an APMS contract. The contract term will be 3 years, with the option to extend for up to a further 36 months, with services scheduled to commence on 1st September 2024. The annual financial envelope is expected to be approximately £536,000.

In addition to providing primary care services, the provider is required to deliver the STAR project. This service supports Rough Sleepers and those at risk of Rough Sleeping into Drug and Alcohol Treatment Services (STAR) in Exeter only. It is a non-recurrent service commissioned through NHS Devon on behalf of Devon County Council. The service is due to expire on 31 March 2025, however, there is a possibility the service may be extended. The annual funding is £154,416 for 2023/24 and indicated to be similar for 2024/25 (not yet confirmed).

NHS Devon is aware that the current provider in Exeter also delivers services under a subcontracting arrangement with Humankind. This includes Opioid Substitution Therapy, Dried Blood Spot Testing and to deliver Buvidal to appropriate clients. This service requires specialist skills and is funded separately for Exeter only.

Interested parties must be eligible to hold an APMS (Alternative Provider Medical Services) contract and appropriate CQC registration. The key requirements, that providers will be expected to meet are:
• Demonstrable understanding of the patients experiencing homeless/complex lives
• Demonstrable evidence of delivering health care services to a similar group
• Delivery within the budget
• Demonstrable ability to provide high quality primary medical services
• Evidence of managing service improvement and delivering change in a challenging environment
• Evidence of partnership working with system organisations and other agencies, voluntary and statutory groups
