NHS Devon (the Commissioner) sought a provider to provide safe, effective and agile care to patients who are discharged under the Discharge to Assess (D2A) scheme and are admitted into a residential/nursing home. This will also include end of life patients.

The contract term is 1st October 2022 to 30th September 2023 with the option to extend for a further 6 months.
